Registration as service providers offering qualified certificates

This form is used to apply for registration of the provision or issuance of qualified certificates to the public as referred to in Section 2.1 of the Dutch Telecommunications Act as amended in the Electronic Signatures Act (Dutch Bulletin of Acts and Decrees 199, 2003). Completion of this form facilitates quick processing of the registration and also serves as a guide in compiling the information file. The information file is a collection of information regarding the setup of the services provided to the public by a provider of qualified certificates. The information in this file forms the basis of the registration as well as future regulation of compliance with the legal obligations. The contents of the file consists of the answers to the questions and the relevant appendices.
